Osteoarthritis / Degenerative Joint Disease

It often begins so gradually that many people barely notice it at first — a mild stiffness when rising from a chair in the morning, a dull ache in the knee after a long walk, or a grinding sensation while climbing stairs. Activities like kneeling, squatting, or sitting cross-legged may slowly start to feel uncomfortable and are often dismissed as part of “getting older.”

Over time, the symptoms tend to progress almost imperceptibly. Morning stiffness lasts longer, the ache becomes more persistent, and pain may be present not only after activity but also during movement and sometimes even at rest. Stairs become more difficult to manage, walking distances feel shorter, and daily activities such as gardening, dancing, playing with grandchildren, or morning walks begin to feel increasingly challenging.

This is the progressive reality of osteoarthritis — the most common joint condition worldwide, affecting hundreds of millions of people across all age groups and lifestyles. Despite its prevalence, osteoarthritis is often misunderstood and inadequately managed, with many patients advised only to “manage the pain,” rely on medications, or wait until joint replacement becomes necessary.

At Activelife Physiotherapy, we fundamentally reject this passive approach. Evidence shows that structured, expert physiotherapy is one of the most effective interventions for osteoarthritis management, helping improve pain, mobility, strength, and overall quality of life — safely and sustainably, without the risks of surgery or long-term medication dependence.

You do not have to simply endure osteoarthritis. With the right approach, you can actively manage it and continue living well.

Osteoarthritis (OA) is a chronic, progressive condition characterised by the gradual deterioration of the articular cartilage — the smooth, glistening tissue that covers the ends of bones within a joint — along with changes in the underlying bone, joint lining (synovium), and surrounding soft tissues.

For decades, osteoarthritis was described simply as “wear and tear” — the gradual breakdown of cartilage through years of joint use. While this is partly true, modern research shows that osteoarthritis is a far more complex biological process involving multiple interacting changes within the joint.

  • Cartilage matrix degradation caused by enzymes released by stressed chondrocytes
  • Subchondral bone remodelling — thickening and hardening of the bone beneath damaged cartilage
  • Osteophyte formation — growth of bony spurs at joint margins as the body attempts to stabilise the joint
  • Synovial inflammation — low-grade inflammation in the joint lining that increases pain sensitivity and accelerates cartilage breakdown
  • Muscle weakness and inhibition — reducing joint support and increasing mechanical stress on damaged structures
  • Neurological sensitisation — heightened pain response from the nervous system that amplifies symptoms beyond structural damage

This biological complexity is important because it shows that osteoarthritis is not simply a “wear and tear” problem that can only be solved with joint replacement. Instead, it is a whole-joint and whole-body condition that can respond significantly to the right conservative care.

At Activelife Physiotherapy, we focus on evidence-based rehabilitation strategies that address pain, movement, strength, and function — helping patients manage osteoarthritis effectively and improve quality of life.
